Online Medical Records Request

When submitting a request online, please complete all required fields. You will need to upload a copy of your government issued photo ID.

Requests will be processed within five to ten business days of receipt. Requests are fulfilled by our medical records partner, Verisma.

In order to release medical records, we require the completion of an HIPAA compliant release of information form that must be mailed, faxed or delivered to the hospital.
